Coming out of players' nightmares is the most famous monster from Silent Hill as a whole, Pyramid Head. He is a brutal executioner with an unseen face that's covered by a metal pyramid. The only semblance of a mouth can be known from a tongue-like appendage that Pyramid Head can bring out by peeling up the corner of the front plate of the pyramid.
Readers should note going forward that this analysis of the symbolism behind Pyramid Head in Silent Hill and his actions contain heavy spoilers for the entirety of Silent Hill 2. As always, this is up to interpretation, and those who wish to not be spoiled are not advised to go forward.
James Sunderland is the main protagonist and playable character of Silent Hill 2. After the death of his wife, Mary Sunderland, James receives a letter from her postmortem that asks him to meet her in Silent Hill. Being that Silent Hill was a favorite vacation spot for the two of them, James travels there and finds twisted horrors awaiting him. While trying to find Mary, James encounters other people who are looking for things or people in Silent Hill as well as a woman named Maria that looks exactly like Mary.
The main difference between the two however is that Maria has a more outgoing personality and fashion choices while Mary was very modest in nature. She ends up becoming James' companion through his journey in Silent Hill. Eventually, it is revealed that the letter James has been holding the entire time has been blank and that he in actuality had killed Mary very recently before the game had started and that Mary's body was back in James' car trunk at the start of the game.

All of those creatures killed the parents. We tried to make it fit in. I created Valtiel because I wanted to relate the works of the series through a creature. In the religion of Silent Hill, Valtiel exists as one who is close to God , and Pyramid Head takes the shape of Valtiel's reason for existence. Pyramid Head acts as a metaphor for James, as he murders Maria, a manifestation of James' lust towards his dead wife, multiple times in the game. He is also forced to lug James' guilt everywhere he goes, depicted through the Pyramid.
The scene where Pyramid Head rapes the mannequin displays James' sexual desire, as many of the enemies in the game represent his or another inhabitant of Silent Hill's psyche; the mannequin and nurses symbolizing his sexual deprivation the Doorman is Angela's father, the straight-jacket enemies being James' knowledge of his insanity, etc. And finally, James' greatest fear and enemy within the game is Pyramid Head, symbolizing his terror of the guilt he's blocked from his mind.
Pyramid Head also appears in Silent Hill: Homecoming , once again reprising his role as the executioner and manifestation of guilt over murder. His design is borrowed from the one made famous by the Silent Hill film, but this time around he only makes appearances in non-interactive cutscenes and is refereed to as being the Bogeyman. Many fans were disappointed by the addition of Pyramid Head in the game, and dismissed it as failed fan-service. Unlike his Silent Hill representations, Pyramid Head's biography for the game depicts him as being a demon from hell.
The Pyramid Head made its way into the movie industry in the Silent Hill titular movie adaptation, which premiered in and was directed by Christophe Gans. He is the film's most noticeable, threatening and dangerous enemy and its revamped movie design was later imported into Silent Hill: Homecoming. Pyramid Head has also been confirmed to make an appearance of the upcoming film 'Silent Hill: Revelation.

Pyramid Head functions as both an executioner of Maria, and the constant tormentor and punisher of James. The fact that he's seen molesting various monsters could be symbolic to James Sunderland's sexual frustration, seeing that James' wife has been deceased for 3 years.
In the films, Pyramid Head's design is noticeably different from that which is depicted in the games. Christophe Gans, the director of the first film, noted that despite the name, Pyramid Head's helmet is actually "a basin and not a triangle at all".
Apparently, "it was impossible to make an authentic replica of the shape and actually have a real person to move under it. Pyramid Head's "very deformed body" in the game was also altered into a "tall, powerful character a little like the Warrior God in Stargate ". Gans has also stated that Pyramid Head's appearance is conjured from a female perspective. He was created by Alessa to punish the cultists for what they have done to her. After Rose fell through the window to the school's courtard Pyramid Head appeared from the door next to her with a huge swarm of Creepers.

He can be unlocked by obtaining at least bronze in every event in the Single Event mode.
The game also mentions that he went on "a brief but enjoyable killing spree" in Hollywood, a reference to his film appearance. He could be bought with or without his signature weapon. In the story mode for the Grand Prix mode, he appears to the Bomberman Brothers along with Dracula and Anubis to judge their strength.
